Patients suffering from diagnosed, covered chronic medical conditions who have limited financial means (when program is accepting enrollment) looking for assistance with FDA approved medications, and income level at or below 500% of the Federal poverty leve. Patient under the age of 18 can be accompanied by up to 2 parents or legal guardians, patient age 18 may be accompanied by 1 medically-necessary caregiver, be able to board and exit the aircraft and sit upright in seat or wheelchair, not on any life support systems, meet airline approved portable oxygen criteria, going to or from recognized treatment center, have self arranged ground transportation, self pay baggage fees, and meet income guidelines. Qualifying applicants may receive assistance as often as needed.
