This Section of the Medical Home Portal aims to provide primary care physicians and other health care professionals with information, tools, and resources to enable ongoing improvement in
  • care and outcomes for their patients,
  • providing patient- and family-centered care, and
  • appropriate compensation for the extended or additional services involved in providing a comprehensive Medical Home.
The Building a Medical Home section provides an overview of what a Medical Home means to providers and to families and its subpages offer detail about:

  • how to provide Care Coordination for those patients who could benefit from it

  • where to can find training materials and other resources to guide your efforts to improve the “medical homeness” of your practice
The Screening and Prevention section offers guidelines and resources for screening for developmental, sensory, nutritional, and other health-related problems to allow early intervention for or prevention of chronic conditions and their complications.
Technology Reviews offers overviews of devices, equipment, and other interventions aimed at assisting children with chronic conditions and improving their health and functional capabilities.
Education & Schools provides information to help you understand how educators, schools, and school systems can help your patients and how you can help schools in assuring appropriate services are provided to your patients. Transition Issues provides an overview of the numerous transitions CYSCHN and other children face and that will go more smoothly with thoughtful planning and awareness of availalble resources (or lack thereof).
