Screening & Prevention

The Screening and Prevention section of the Medical Home Portal provides information and resources to enable clinicians to:
  • Understand available screening tools and tests
  • Screen effectively for inborn and acquired conditions
  • Intervene appropriately when screening uncovers a problem or risk
  • Recognize the populations and identify patients at risk for preventable conditions
  • Initiate and monitor preventive interventions for at-risk patients
  • Collaborate with community agencies and other professionals in screening and prevention measures
