Services Provided: Child Health Evaluation and Care (CHEC) is the name of the Utah Medicaid benefit for children (known elsewhere as EPSDT). CHEC offers: Well-child visits, Immunizations; Vision tests, Hearing tests and Dental visits; Lab tests, Local Health Department nurses who help you understand how important CHEC visits are and will help schedule CHEC visits, Medically necessary services, including Glasses, Hearing aids, Therapy (Speech, Physical, Occupational or Counseling), Assistive technology such as a special wheelchair or tools to make daily life easier, and Reimbursement for travel to go to CHEC appointments (855-563-4403).
Early and Periodic Screening, Diagnostic and Treatment (EPSDT), the child health component of Medicaid. Children under age 21 who are enrolled in Medicaid are entitled to EPSDT benefits, which States must cover with preventive and treatment services.
