Eligibility: Ogden, Vernal, Logan residents with children ages birth to 18. Sliding scale based on income.
Services Provided: Evaluation clinics for children with special health care needs (CSHCN) Child Development Program.
Evaluations are provided by multidisciplinary teams which travel to Ogden. Clinics feature specialty assessment providers in the following areas:
Developmental Pediatrics
Social Work
Speech and Language
Other pediatric evaluations may include: physical and occupational therapy, neurology and genetics.
Satellite CSHCN nurses and staff are available.

CSHCN Satellite Clinic, Itinerant Physician Services, Traveling Clinics
Provider Profiles: Ogden traveling clinics occur 2nd and 4th Wednesday of each month. Team members travel to each clinic site to provide services.

Clinic for Neonatal Follow-up Program also in Ogden

The University of Utah has assumed responsibility for clinical services formerly provided by the Bureau of Child Development, Utah Department of Health.
