Services Provided: The Neurology Clinic, of which Muscular Dystrophy Clinic is part, accepts new patients on a referral basis only. Please have your referring physician's office fax the following information to our new patient schedulers: 1) a written referral outlining the neurological symptoms/diagnosis to be evaluated; 2) clinic notes and pertinent lab and radiology related to the diagnosis; and 3) a demographic outline: including the patient's permanent address, home and office telephone number, insurance information, and a social security number. Each case is evaluated by the proper physician.

Eligibility: Patients seen at this clinic have a diagnosis that results in related neuropathy and myopathy, such as muscular dystrophy. New patients must first be seen in the Outpatient Clinic, for referral to the Neuromuscular Clinic.

The Neuromuscular Clinic is a monthly, multispecialty clinic held on the third Tuesday afternoon of each month. Orthopaedics, developmental pediatrics, neurology, nutrition, PT/OT, wheelchair & seating, orthotics, genetics counseling, social work and care management are each represented to assess and evaluate patients.

Specialty clinics allow for patient families to benefit from the variety of experts, and are an opportunity for families to come together and network with other families dealing with similar challenges.
