Your Medical Home - Home Visiting Services

By Al Romeo, R.N., Ph.D. Home visiting services are designed to support families with pregnant moms or young children. Depending on the home visiting model that local programs use, organizations may teach families parenting skills, provide developmental screenings, and link families to community services. With limited funding, the local organizations often limit eligibility to fist-time mothers, low-income families, families with newborns with special health care needs, and families with histories of substance or child abuse. The home visiting model determines when the first visit is offered, how long the visits last, how frequent the visits are, and when the visits will end. The organizations may utilize professionals, paraprofessionals, or peer-volunteers to provide various services.
For more information on home visiting service models and programs, see the Additional Early Services page on the Medical Home Portal.
