- Families can get help with care coordination, finding community resources and providers, or finding content on the Medical Home Portal website.
- Medical home care coordinators, primary care clinicians, and parent partners are encouraged to call when they need help with challenging situations affecting families
Individuals in Utah can call or email to be connected with staff of the Integrated Services Program at the Utah Department of Health’s Bureau of Children with Special Health Care Needs (CSHCN) during regular business hours. Messages that are received after hours will be returned the following business day.
The Integrated Services Program is a no-cost program to help coordinate care for children and youth with special and complex needs and their families with needed specialists and services, in partnership with their primary care provider. The Utah Department of Health's Bureau of Children with Special Health Care Needs is a partner of the Medical Home Portal.
