Services Provided:	Serving the State of Utah and Intermountain Region. The Neurology Clinic (Muscular Dystrophy Association Clinic is part) accepts new patients on a referral basis only, and thus all appointments should be orchestrated through a referring physician. Please have your referring physician's office fax the following information to our new patient schedulers: 1) a written referral outlining the neurological symptoms/diagnosis to be evaluated; 2) clinic notes and pertinent lab and radiology related to the diagnosis to be evaluated; and 3) a demographic outline: including the patient's permanent address, home and office telephone number, insurance information, and a social security number This information may be faxed to the dedicated New Patient Scheduling line (801-585-2746). When the aforementioned information is received, it will go through a review process to ensure that each case is evaluated by the proper physican.
