Services Provided:	1. Forms - Online Court Assistance Program (OCAP) forms as well as modified forms in Microsoft Word format. Forms come in fill in the blank, multiple choice, and sample formats. Forms cover divorce and paternity/custody issues including - answer and counter petition packets, motion for temporary relief packets, stipulation and final documents packets, mailing certificates which includes Attorney General, and petition for paternity packets. It is strongly encouraged to electronically save all of the forms in case the matter is contested or pages or documents are lost before filing. 2. Computers, Internet, Online Court Assistance Program (OCAP), and a printer. Blank disks will be available for $1.00 from the Clinic or you may bring your own formatted disks. 3. Assistance in preparation of documents to be filed. 4. Assistance and information on procedural questions. 5. Notary Public.
