Services Provided:	1. Employment - assist with transitional employment, getting and maintaining jobs with support. 2. Education - opportunity to earn a high school diploma or GED. Tutoring available. 3. Day program - a place to develop friendships, participate in a work-ordered day, work side by side with peers and staff to complete daily club goals. 4. Assist members in obtaining safe, affordable housing. 5. Transportation - provide transportation to and from work, medical appointments, etc. as needed.
Eligibility:	Adults, 18 years or older, suffering from any major mental illness identified as being severely & persistently mentally ill (SPMI), resident of Carbon, Emery, or Grand Counties, with a referral (see Referral Comments).
