STD testing and physician referral. 7. Tuberculosis (TB) Control. 8. Cardiovascular Program. 9. Birth/Death Certificate issue. 10. CHEC physical examination. 11. SIDS Program. 12. Infants (0-5) Home Visitation Program. 13. Early Health Intervention Program, children (0-2). 14. Targeted Care Management. 15. Substance Abuse referral. 16. Family Planning Counseling (issues oral contraceptive pills, condoms, and diaphragm vouchers from referred physician). 17. Environmental Health Program (a) Septic tank permit (b) Water analysis (c) Food handlers permit class. |
