2. Inpatient and outpatient surgery. 3. Laboratory, diagnostic imaging (x-ray, mammogram, etc.), sleep lab testing, respiratory therapy; pulmonary function testing for most ages but may not bill if child is unable to perform PFT properly. 4. 7 Physical therapy clinics - located in North Logan, Preston, Perry, Providence, and Tremonton. 5. First Care Program - occupational health program for employers. |
