| Services Provided: |
1. Employment - assist with transitional employment, getting and maintaining jobs with support.
2. Education - opportunity to earn a high school diploma or GED. Tutoring available. 3. Day program - a place to develop friendships, participate in a work-ordered day, work side by side with peers and staff to complete daily club goals. 4. Housing - manage 2 housing projects - 1 independent with 6 apartments, 1 supported housing to accommodate up to 9 individuals. Assist members in obtaining safe, affordable housing. 5. Transportation - provide transportation to and from work, medical appointments, etc. as needed. |
| Eligibility: | History of mental illness. Must be an active client of Four Corners Behavioral Health. |
