Immunizations Hours - 2 - 4 p.m., Tue. g. Infants (0 - 5) Home Visitation Program. h. Medicaid, PCN, CHIP, & Baby Your Baby eligibility. i. Communicable Diseases - including Sexually Transmitted Diseases (STD) and HIV/AIDS testing and counseling. j. Substance Abuse referral. k. Targeted Care Management. l. Tuberculosis (TB) Control. m.
