Services Provided: 1. Medical and dental care, including primary health and preventive dental care, CHIP/Medicaid application assistance, health education presentations and immunization services. New intakes during morning only, 8-30 a.m. to 11-30 a.m.
2. Diabetes program offers individual and family consultations and diabetes educational presentations.
3. Healthy lifestyles offers health promotion and disease prevention, weight management, blood pressure and LDL screening, counseling and exercises.
4. HIV and STD testing as well as prevention and educational presentations.
5. Adult substance abuse care offers intensive outpatient treatment, individual and group cognitive/behavioral therapy, support groups and referral services.
6. Youth substance abuse prevention program offers individual, family, and group therapy; therapeutic and educational services; substance abuse support services; foster care support; and family preservation.
7. Case management.
8. Counseling, parenting and family support programs.
9. Information and referral provided.
10. Elders support group offers community events open to all.
11. Statewide Coalition for Substance Abuse and Tobacco.
12. Food pantry serving zip codes 84115, 84105, 84108, 84106, and 84109. Food pantry open to the general public, all other services are for Native Americans only.
Eligibility: Utah residency (utility bill or mail except personal letters), photo ID, Social Security cards for all listed on the intake form, tribal ID or Certificate of Indian Blood (Native Americans only), and verification of income (three most recent pay stubs).
